FINANCE DOCUMENT TYPES

Following is listing of document types and a full description of each.

NOTE: Do not highlight information on documents. The highlighted area will be blackened during imaging and that text will not be read by scanner.

Agent Correspondence	Any correspondence between agents that will not affect the rating of a shipment.
Additional Service	Any additional services performed
ASPOD DD6191	Additional Services report or **copy of the DD 619-1 form
Auto Inventory	Inventory of autos, boats, and motorcycles
Doc Transmittal	Atlas’ document transmittal form
Electronic Survey	Electronic Survey of inventory
Hi Value Inventory	Inventory of High Value
Incident Report	Report of Incidents
Inventory	Inventory of goods shipped
Misc Document	Any support document not otherwise listed
Origin Certification Report	Govt Only-Electronics Inventory Condition Form
Origin Dest Service Report	Pack/Unpack Report
Pending BL	Bill of Lading not signed at final delivery or storage
Purchase order	Authorization of Charges
Rec Vehicle Report	Recreational Vehicle Report
Rec Vehicle Ryder to Inventory	Recreational Vehicle Ryder to Inventory
Repair Bill	Repair Bill
Repair Bill Agent	Repair Bill
Signed BL or POD	Delivery Bill of Lading signed to residence or storage (Proof of Delivery) - can be any document signed at residence or storage at destination
Table of Measurements Survey	Cube Sheet
Third Party Hlg Inv	Third party (non-agent) hauling invoice.
Third Pty Dest Inv	Destination 3rd party invoice and *debris pickup
Third Pty Orig Inv	Origin 3rd party invoice
Weight Tickets	Weight Tickets or Certified Weight Statement

* If the debris pickup is recorded on the Additional Services report scan as Additional Service document, or if the debris pickup is recorded on the Packing Services report scan as Packing Service document.

**Note: Original Govt/Military documents must still be sent to AVL Canada
